The layout

What the Two Band Header layout looks like

An “Earnings Statement” title, with your logo if you upload one, sits at the top. The first colored band labels Company, Employee and the stub number, over the two name-and-address blocks. The next bands label the key facts: SSN, pay date, pay period, pay frequency, employee ID, filing status and how the pay was made. A fact you leave blank drops out, and the rest share the width.

The table below lists earnings with rate, hours, current and YTD, then deductions split under Taxes and Other Deductions sub-headings in the same columns. A summary grid closes the page: This Period and Year to Date, across gross taxable pay, deductions and net pay. A line under it explains any non-taxable reimbursement.

Who this template suits

Anyone who wants the familiar look of a printed payroll statement: labeled boxes, one table and a summary grid. Because every fact has a fixed, labeled place, it suits people who compare several stubs side by side, salaried and hourly alike.

Overview

About this two band header earnings statement template

The Two Band Header Earnings Statement Template is a classic grid layout: full-width info grid, earnings and deductions side by side. Colored header bands over the company, employee and pay details, grid tables, and a this-period and year-to-date summary grid.

From top to bottom it runs title, with your logo when you add one, band: company, employee and stub number, bands: SSN, dates, frequency, employee ID, filing status, paid by and earnings table, and then the rest of the stub. You fill it in once in the builder, the taxes and totals are worked out for you, and the finished earnings statement downloads as a PDF.

What each part of the Two Band Header shows

1. Title, with your logo when you add one

It opens the Two Band Header. The document’s name and reference: the title the Two Band Header prints, with the stub number and, where the layout has them here, the pay frequency and filing status. A unique stub number makes each copy easy to refer to.

2. Band: company, employee and stub number

The second block on the Two Band Header. Payer and payee together. The Two Band Header prints the business with its address and EIN next to the person paid, with an ID and a masked SSN; in contractor mode these read as payer and contractor on layouts that support it.

3. Bands: SSN, dates, frequency, employee ID, filing status, paid by

The third block on the Two Band Header. The dates that tie the stub to a paycheck: the period worked and the pay date, with pay frequency and filing status where the Two Band Header shows them. The pay date should match the deposit.

4. Earnings table

The fourth block on the Two Band Header. Earnings lines: hours × rate for hourly pay, one line for a salary, and separate lines for overtime, bonuses or tips. On the Two Band Header, the total of these lines is the gross pay.

5. Deductions under Taxes and Other Deductions

The fifth block on the Two Band Header. What came out of gross pay: federal income tax, Social Security, Medicare, state and local tax where they apply, then your own deductions such as a health plan or retirement. The builder estimates the taxes; every line stays editable.

6. Summary grid: this period and year to date

The sixth block on the Two Band Header. The year so far. On the Two Band Header, YTD totals sum every pay date in the calendar year up to and including this one, so consecutive stubs should step up by each period’s figures.

7. Reimbursement line and memo note

It closes the Two Band Header. Reimbursements and other non-taxable pay are kept apart from wages, so they raise net pay without changing the taxable gross.

Colours

Two Band Header colours

The Two Band Header comes in Cerulean, Tide blue, Ink, Lagoon, Navy and Black & white, with Cerulean as its standard colour. The colour you choose recolours the accents (header bars and section rules); the figures and layout stay the same, and every colour is included in the price.

Document type

Why the Two Band Header is called an earnings statement

“Earnings statement” is the title many payroll departments print on the same document others call a pay stub or pay slip. The Two Band Header is drawn as a statement (a titled page with the period and the figures laid out formally), but it holds exactly what a pay stub holds: who was paid, for which period, gross pay, each tax and deduction, net pay and the year to date.

Best use

When to choose the Two Band Header

Traditional payroll readers

The Two Band Header keeps the classic grid order (details across the top, earnings beside deductions), the layout most people already know from payroll stubs.

Employees on payroll

The Two Band Header prints the full set of withholding lines (federal, Social Security, Medicare, state) for a W-2 employee’s stub.

A formal statement

The Two Band Header’s statement title and layout suit a stub that is filed with other paperwork or handed to someone else.

Several pay periods in a row

The Two Band Header prints year-to-date figures in “Summary grid: this period and year to date”, so a run of consecutive stubs shows the year building up period by period.

They share the same overall order; what sets the Two Band Header apart is colored header bands over the company, employee and pay details, grid tables, and a this-period and year-to-date summary grid.

Print and PDF

Printing and reading the Two Band Header

The full-width grid fills a letter page edge to edge, so it prints clearly in black and white and stays readable when photocopied or scanned. The PDF is made from the same layout you see in the preview, so what you check is what you get.

Need several periods? The multi-stub builder makes consecutive stubs in this layout with year-to-date totals carried forward, and the sample pay stub walks through every line with real figures.

Checks on a finished Two Band Header

  1. In “Earnings table”, rate × hours equals each line, and the lines add up to gross pay.
  2. In “Deductions under Taxes and Other Deductions”, every tax and deduction you expect is listed once, with its correct name.
  3. In “Summary grid: this period and year to date”, net pay equals gross pay minus total deductions (plus any reimbursement) and matches the deposit.
  4. In “Summary grid: this period and year to date”, year-to-date totals include this period and step up correctly from the previous stub.
  5. Names, addresses and the pay date are spelled and dated as on your records.
