A dark header runs across the top, with the “Earnings Statement” title and the pay period on the left, and your logo in a white chip and the stub number in a pill on the right. Three columns follow under underlined labels: company information, employee information and pay details, each listing only the values you fill in.
Earnings (description, hours, rate, amount and year to date) sits beside taxes and deductions, each closed by a bold total. When you enter year-to-date figures, a tinted strip shows gross pay, taxes and deductions, and net pay for the year so far. A summary block lists gross taxable pay, total taxes and deductions, and any non-taxable pay, and a solid bar at the foot carries net pay.

Best for
Small businesses and contractors who want a crisp, modern document that still reads like a payroll statement. The dark header and the net pay bar frame the page, so the key figures stand out whether the stub is printed or opened as a PDF on a phone.
Overview
The Pill Header Earnings Statement Template is a classic grid layout: full-width info grid, earnings and deductions side by side. A dark header with the stub number in a pill, three underlined info columns, earnings beside deductions, a year-to-date strip and a solid net pay bar.
From top to bottom it runs dark header: title, pay period, logo and stub number pill, company, employee and pay details columns, earnings with hours, rate, amount and YTD and taxes and deductions with the total, and then the rest of the stub. You fill it in once in the builder, the taxes and totals are worked out for you, and the finished earnings statement downloads as a PDF.

Section by section
It opens the Pill Header. The Pill Header names the business here: company name, address, and the EIN and phone if you give them. A logo, when you upload one, sits in this block.
The second block on the Pill Header. Both sides of the payment in one place: the payer (company name, address, EIN) and the person paid (name, address, ID and the SSN masked to its last four digits). On the Pill Header they sit together, so who paid whom reads in one glance.
The third block on the Pill Header. Earnings lines: hours × rate for hourly pay, one line for a salary, and separate lines for overtime, bonuses or tips. On the Pill Header, the total of these lines is the gross pay.
The fourth block on the Pill Header. What came out of gross pay: federal income tax, Social Security, Medicare, state and local tax where they apply, then your own deductions such as a health plan or retirement. The builder estimates the taxes; every line stays editable.
The fifth block on the Pill Header. The year so far. On the Pill Header, YTD totals sum every pay date in the calendar year up to and including this one, so consecutive stubs should step up by each period’s figures.
The sixth block on the Pill Header. The short answer before or after the detail: gross pay, total deductions and net pay together. On the Pill Header it is the quickest place to confirm the stub matches the deposit.
It closes the Pill Header. The take-home figure. The Pill Header gives net pay its own block so it is the first number a reader finds; it equals gross minus deductions, plus any reimbursement.

Colours
The Pill Header comes in Slate & green, Navy, Harbor blue, Spruce, Aubergine, Brick, Ink and Black & white, with Slate & green as its standard colour. The colour you choose recolours the accents (header bars and section rules); the figures and layout stay the same, and every colour is included in the price.
Document type
“Earnings statement” is the title many payroll departments print on the same document others call a pay stub or pay slip. The Pill Header is drawn as a statement (a titled page with the period and the figures laid out formally), but it holds exactly what a pay stub holds: who was paid, for which period, gross pay, each tax and deduction, net pay and the year to date.
Best use
The Pill Header keeps the classic grid order (details across the top, earnings beside deductions), the layout most people already know from payroll stubs.
The Pill Header prints the full set of withholding lines (federal, Social Security, Medicare, state) for a W-2 employee’s stub.
The Pill Header’s statement title and layout suit a stub that is filed with other paperwork or handed to someone else.
The Pill Header prints year-to-date figures in “Earnings with hours, rate, amount and YTD”, so a run of consecutive stubs shows the year building up period by period.
Compare
The Pill Header belongs to the Classic grid family, with Default, Professional Classic, Refined Blue and Minimal Professional. They share the same overall order; what sets the Pill Header apart is a dark header with the stub number in a pill, three underlined info columns, earnings beside deductions, a year-to-date strip and a solid net pay bar.

Print and PDF
The full-width grid fills a letter page edge to edge, so it prints clearly in black and white and stays readable when photocopied or scanned. The PDF is made from the same layout you see in the preview, so what you check is what you get.
Need several periods? The multi-stub builder makes consecutive stubs in this layout with year-to-date totals carried forward, and the sample pay stub walks through every line with real figures.

How it works
Open the builder with the Pill Header already selected (the “Use This Template” button does this).
Enter the company and the person paid; the business prints in the “Dark header: title, pay period, logo and stub number pill” block.
Add the pay period, pay date and each kind of pay; the lines fill “Earnings with hours, rate, amount and YTD”.
Check the estimated taxes and add your own deductions (they print in “Taxes and deductions with the total”); every amount can be edited.
Pick a colour (Slate & green, Navy and Harbor blue or another), preview the whole stub, then download the PDF.
